Subsequent to the massage, the therapist post their comments and notes, and the forms are then placed in a secure locked filing cabinet to ensure the clients privacy. When the client returns for their next massage, the questionnaire is pulled and reviewed with the therapist, to discuss any improvements or additional issues that need to be addressed.
